RELEASE OF LIABILITY
_______ I understand that injuries are a natural possibility in any athletic endeavor and will assume full responsibility for any injuries incurred at TranscenDance Studios.  Therefore, I agree to hold TranscenDance Studios harmless of and from any and all claims, demands, liabilities, losses, lawsuits, judgements, costs, damages, and other expenses of any kind or nature whatsoever.

RESCUE SQUAD AUTHORIZATION
_______ I hereby authorize that, in the event of a medical emergency, a rescue squad may be contacted if TranscenDance Studio Staff deems necessary.

TranscenDance Studios Policies

TUITION DUE DATES - Tuition for the Monthly Auto-Debit Option is withdrawn the 5th of every month.  Tuition for Half-Season Payment Option is due the first day or classes and February 1.  See the separate Tuition Policies & Discounts Sheet for complete information on tuition, costume fees, registration fees, and withdrawals. 

REFUNDS - Refunds will be issued only with a medical excuse (physician’s note required) or to those moving out of area or state. Registration fees are not refundable. There will be a $25 administrative fee charged for processing any refund. No exceptions!

PAYMENT - Payment is accepted on location in cash, personal check, or credit card.  Make checks out to TranscenDance Studios.  Credit card payments can be made through Parent Portal from the TDS web site:  www.TranscenDanceStudios.com.  

DROP/ADD/CHANGE - The studio must be informed of the intention to change a class. Any withdrawal from class must be made at least 7 days prior to the first of the upcoming month to avoid an additional month charge on the Monthly Auto-Debit Payment Option.  Parents must fill out a Withdrawal Form available during studio business hours at our reception desk.  If a Withdrawal form is not completed, accounts will continue to be charged and held accountable for all charges and fees.  Withdrawals may only be made in person through a Withdrawal Form. 

NOTICE OF FEES - There will be a $25 charge for each late payment. Payments are considered late when one month past due. There will be a $25 charge for all returned checks (insufficient funds or other).Re-deposited checks will incur a $10 fee.

CANCELLATION - All classes are subject to cancellation, with full refund, if minimum student registration is not met.

PLACEMENT - The first three weeks of classes are an evaluation of each student’s ability. In all instances, qualification for a particular class will always be at the instructor’s and the studio director’s discretion.

ARRIVAL - Be prompt.  All Pre and Beginning Level students must be signed in by their parents with their class assistant.  FFC Members may sign in their children at either the TDS reception desk in the lobby or in the dance studios on the second floor.  Non-members must sign in their children at the TDS reception desk in the lobby per FFC policy.  The class assistant will escort children between the studio rooms on the second floor and the TDS reception desk on the first floor.  Please do not stop a class en route to the studio rooms. 

We suggest that older dancers arrive 15 minutes early to warm themselves up and change. Please do not drop dancers off for extended periods of time before or after their class. We do not have the facilities to supervise.  Intermediate and Advanced dancers must check in with the TDS desk staff before class.

LATE ARRIVAL - Any student arriving more than 15 minutes late will not be admitted to class. For your safety and to receive full benefit from your class, please arrange for a make-up class if you expect to be very late.  All Beginning Level students that arrive after TDS staff has taken their class upstairs must be escorted upstairs by their parent after checking in with the TDS reception desk.  Chronic late arrival may be grounds for dismissal from class without a refund. 

PICK UP - Pre and Beginning Level students must be signed out with the class assistant by their parent.  FFC members may sign in and pick up their child from the dance studios on the second floor, or wait until the group has moved to the reception area on the first floor.  Non-members must pick up and sign out their child from the class assistant in the reception area on the first floor.   Parents must come into the reception area to pick up your child if he/she is under the age of 10 no matter what level they are enrolled in.  We do not want young children running across streets or between parked cars to meet you; this creates a very dangerous situation. 

Children under the age of 14 can not meet you elsewhere in the club unless they are members, and it is during Family Hours.  Please the the Supervision Policy. 

SUPERVISION - All Pre and Beginning Level dancers must be supervised at all times once in the building.  This is by the parent before and after class, and by the class assistant on the way up, down, and during class.  TDS desk staff will not supervise your Pre or Beginning Level child before or after class.  If you are running late, call to let the TDS desk staff know. 

The only place a dancer between the ages of 10 and 13 may be unsupervised is walking between the dance studios and elevators on the second floor where they are in sight of the FFC desk staff.  Children between 10 and 13 may wait for parents in the first floor lobby when TDS desk staff is present.  Dancers 10+ may wait before class, after class, or in between classes in the dance studio with the teacher present.  Dancers may observe class and must not interrupt or participate.  Dancers 10-13 must take a parent or 14+ year old buddy with them to the bathroom. 

RESTROOM USE - Parents of Pre and Beginning Level students are responsible for making sure their children use the restroom before class.  If the need arises during class, one of the class assistants will take the child to the restroom.  Any opposite sex children/assistants will go down to the Kids Club restroom on the first floor.  Same sex children/assistants may use the locker room restrooms on the third floor.  Intermediate and Advanced level dancers must use the restroom before or after class. 

HOLIDAY & BREAK SCHEDULES - The season calendar can be found in the Student Handbook and on our web site:  www.TranscenDanceStudios.com.  Any changes to this calendar will be sent home by email or paper handout. 

DRESS CODE - All students are required to own either a TDS t-shirt or Warm Up jacket.  Dancers must wear their TDS t-shirt or Warm Up jacket at all times they are in the building, unless they are in class.  This allows identification by TDS and Fitness Formula Clubs faculty for safety reason. No exceptions.

CLASS WEAR DRESS CODE - We request that students carefully observe our dress code outlined in the  Student Handbook.

DANCE SHOES - Dancers are required to have the correct shoes within 3 weeks of entering a class. Please label all dance shoes with the student’s last name.

RECITAL PARTICIPATION - All students are invited, but not required, to participate in our annual June recital. This is a fully staged, costumed, professional quality dance program showcasing our students and teachers’ choreography.  Students are required to notify TDS if they will not be participating in recital by the first day of winter break.  Recital participation is assumed for all Students unless otherwise notified. 

RECITAL COSTUMES - See the TranscenDance Studios Tuition Policies & Discounts sheet for Recital Costume Fees.  Due to circumstances beyond our control, we will not order costumes for any student whose account is not current by our winter break (December); therefore, those students will not perform in the June recital.  We can not cancel costume orders after the studio goes on winter break.  If you drop a class or decide not to participate in recital after a costume is ordered, you will be notified and may pick up the costume when it arrives. 

ATTENDANCE - Good attendance is required.  If you miss three consecutive weeks of a class without notification of our desk staff, you will be dropped from that class.  Refunds for long-term injury will not be issued if you fail to notify TDS desk staff within those three weeks.  Good attendance after January when classes begin to work on recital choreography is essential.  Six or more absences between January and June is grounds for removal from recital without a refund of costume fee.  See Recital Costumes.

MAKE UP POLICY - In the case of student absence or one-time class cancellation, students are encouraged to make up canceled classes.  Make ups can be made in any dance style and do not have to be the same style as the canceled class. Students should make up in the same level or within one level below or above their regular class.   Prior experience in other styles should be considered when deciding on the appropriate make up level for alternate styles.  Make ups can not be taken in any company rehearsal (Company Work Out allowed) and will not be issued for any missed company rehearsals.  Please notify reception prior to your make up class.  TDS does not issue refunds due to class cancellation or student absence.

ILLNESS OR INJURY - During class if a student complains of being ill, or sustains an injury and cannot participate, the instructor will send them to the TDS front desk, where the parents will be notified if not on the premises.

CLASS OBSERVATION - Out-of-town guests may request to visit at any time throughout the year.  Parents, friends, or sibling are not allowed to sit and watch during regular class time.  Parent observation weeks will be scheduled throughout the year.  Friends who are interested in dancing at TDS may observe class in place of a trial. 

NO FOOD, BEVERAGES, OR GUM - Only water in studios.

ALL CELL PHONES AND DEVICES - All cell phones and noise-making devices must be switched to silent when on premises.  Ringing phones draw unwanted attention to your bags. Cell phones are not allowed out in the studios, please keep them in your purse or dance bag while in class. The checking of cell phones while in class is in session is only allowed in emergency situations. Such situations must be cleared by the instructor before class.

RESPECT - School age children and teens: please practice respect. Students are not to be socializing in a disruptive manner during the class with other students.  Please honor your teacher and fellow students by giving full attention and cooperation. You will be advised if we feel any student is not well-adjusted to the classroom environment for any reason over time. Also, please remember the studio and gym are not a playground. There is absolutely no running or hanging on ballet bars or gym equipment.
